This form needs to be filled out by coaches/chaperones and submitted to the UPA. If being used for the 2007 NEPSUL tournament, it needs to be to the UPA no later than April 29, 2007.

Medical_Authorization_Form_2007_ (21.82 Kbytes) hot! 2006-03-17
All players under the age of 18 need to have their parents fill out this form, and give it to their coach/chaperone to have on hand at the UPA-sanctioned event.

All players and coaches must sign this form. If a player is under 18, a parent/legal guardian must also sign it. This form is required to join the UPA. For the 2007 NEPSUL, all waivers need to be at the UPA before April 29, 2007.

This form (provided here in two versions: PDF and Excel) needs to be filled out by every team's coach/chaperone and submitted to the UPA by April 29, 2007 in order to be eligible to participate in the 2007 NEPSUL tournament.

NOTE: This paper version is best used for players that do not belong to the UPA, and choose to pay the one time event fee (valid for the event, only). Online rostering is recommended for all UPA-members.
